Must be eligible to Book (Must answer YES to all):
*A City of London Resident (square mile) OR A Manual Worker
*Between the ages of 40-74
*Not Diagnosed or on Medication for Heart Disease, Stroke, Diabetes, Kidney Disease, High Blood Pressure or High Cholesterol
*Not had an NHS Health Check with us in the last 5 years
