Invitation To Unpaid, Informal City Carers

The next monthly City carers’ support group meeting will be on:- Tuesday 12th May 2015 at 11am - 12.30pm

Madeline Makarab CoL Care Navigator & Caseworker Adviser

Will speak about the;- Role of Care Navigators 

•      Provides information and support for City of London residents to maintain their independence and aid with safe hospital discharge.

•       Coordinated health and social care for continuity of care.

•       Liaise with health and social care teams/professionals and link with Adult Social Team

•       Make relevant referrals

•       Sign post to available services and local community resources.

•       Provide emotional support including building new social networks

Sarah Hudson from Friends of City Gardens will visit to promote the community gardening group
